When you first arrive at our office, we like to have you fill out a
patient history profile.
From this form we will gather pertinent
information about you and why you chose to see us. We will review
your Medical Health History, as well as your Dental Health History,
and then perform a thorough dental and periodontal examination.
Medical History:
It is important to know if you have any health
problems that may compromise your ability to have periodontal
therapy. Medical conditions such as diabetes or pregnancy
may make you more susceptible to gum infections. Personal habits
such as smoking can promote periodontal disease, and you my be aware
that there is a distinct relationship between heart attacks
and pre-term births with periodontal disease.
Dental History:
We would like to know what your previous
experiences with dental care have been. Have you had to have a lot
of dental treatment in the past? When was your periodontal disease
first diagnosed? How has it been handled? Are you apprehensive about
treatment? We can address all of these issues to assess your
motivation and commitment to undergo periodontal therapy.
